Question:
What am I doing wrong when studying for exams/tests?

I always study study for exams/tests! I do well on my lab reports and all those mini assignment stuffs but when it comes to exams...its like I know what to do but it also seems like a so much harder concept.

I go through all the questions in my text books, teacher's handouts, everything !!
I study a head of time, make notes, and do practice exam questions as well. Even when i think i did well, i do horrible!
I am in university and it seems as if any exam i do, I just fail. Whether it be physics or psychology! the only thing keeping my mark from completely failing are the assignments/reports. I always ask for help too. I do get anxiety the night before my exam but on the day of my exam I am completely calm.
I am sick and tired of this. I get proper nutrition, proper sleep and exercise regularly.

I really want to do well. All my notes are good too. It's really frustrating ! I need help ! I tried soo many different ways of studying and it all resulted in the same thing. I seemed to know more than my friends even moments before the exam but they all get better marks than me.

Whether it be multiple choice or practice question (physics questions) ... I am failing !

Assuming that you got good marks in assignments,it's quiet unusual you have dropped into the level of fail. I think even though you tell that you do everything right there might be something wrong that you cannot see because you haven't never noticed it.

This might sound absurd but what if you forget the superior level and study to achieve middle level score in exam? Study full time with a friend that hasn't been too close with you, he might be able to find the missing point of yours.

BE COOL.....IF U STUDY FROM THE BEGINNING U WONT FAIL.......WORK HARD..BEACAUSE IT IS STUDIES WHICH MAKE FUTURE AND U WILL BE RESPONSIBLE FOR EVERYTHING

It sounds more like you are a bad test taker, not a bad study-er. Read up on google about how to best take tests like that. Its a skill that can be learned. For example, I can take pretty much any test on any topic and I will pass, without studying.
