I'm trying to terminate my membership with a gym and they need the request put in writing. Also should I say ATTN: Membership services or ATTN: General Manager or ATTN: Billing department?

After. Attn: Membership services -- you can also write on the outside of the envelope "Membership Cancellation". Be sure to keep a copy for yourself.
